How to use this template
- Fill in your name, ID, job and the week starting at the top.
- Each day, write the time you start, the time you leave for an unpaid lunch, the time you come back and the time you finish.
- Total each day: (lunch out − time in) + (time out − lunch in). Put any other unpaid break in minutes and subtract it.
- Add the days. If you are non-exempt in the US, hours over 40 in a workweek are overtime under federal law.
- Sign and date it, and have your supervisor sign if required.
